Tell us about the incident
The main things we need to know are:
- The time, date and location of where the incident occurred
- A short description of the incident
- The police crime reference number*
- The name and number of the police officer who dealt with your case
- The name and address of the police station and police force where the officer is based
* We do not usually accept reports of crimes made to employers, trade-union officials or social workers rather than to the police. We may make exceptions in the case of injuries suffered, for example, in schools, residential homes, psychiatric hospitals and prisons, as long as you made an official report to someone in authority and were willing to have the matter formally investigated.
Tell us about the injuries
The main things we need to know are:
- What your injuries were
- The full name, address including postcode and telephone number of your GP (even if you did not see your GP about these injuries)
- If you had hospital, consultant/specialist or dental treatment for these injuries, full names, addresses with postcodes and telephone numbers for them.
